If you did not attend Festival, or somehow managed to make it past Kevin at Festival (stares at Staff), you will fill out a medical history form and sign your liability waiver at All Hallows, regardless of your anniversary date.
Medical history forms are found in the forms section of the facebook group and on the website in to Info section. If you have had a change in your medical history (new meds, new diagnosis, pregnancy, so on) since you filled one out last, you need to fill a new one at (...)

For example, if you are full time NPCing, you are staying with the full time NPCs.
If you are playing an Ancient, you stay with the Ancients.
